This guide is written for support coordinators and allied health professionals referring participants to Accelerate Care.

What we need to assess fit

You do not need everything finalised before referring. Submit what you have and we will follow up on anything missing.

Useful information includes:

Participant name, age, and suburb within Melbourne's inner and northern suburbs

NDIS plan details including funding category and available budget

Type of support required and approximate hours per week

Any preferences around worker gender, language, or background

Relevant disability, health, or behavioural considerations

Urgency and preferred start date

Your contact details for follow-up

How the process works

Step 1. You submit the referral using our referral form. Takes around two to three minutes.

Step 2. We review it properly. We look at the participant's needs, preferences, and urgency. This is where the matching process starts.

Step 3. We make contact within 24 hours. We will call you or the participant to confirm details and next steps. If anything is urgent, call us directly.

Step 4. We confirm fit. If we are the right provider, we say so and move forward. If we are not, we tell you upfront.

Step 5. We begin matching based on fit, not just availability. Personality, communication style, and the participant's specific needs all factor in.
